Fireman Examinations
In accordance with NFPA 1582, Medical Requirements for Fire Fighters, 2000 Edition, a return-to-duty exam is to be performed on all appropriate candidates who have been absent from duty for a medical condition of a nature or duration that may affect performance as a fire fighter. The fire fighter who has an impairment of their musculoskeletal system must undergo a physical exam and an M/S Evaluation. Examples of conditions requiring M/S Evaluation include but are not limited to:
- Broken bones
- Sprains or strains to the neck, shoulder, elbow, wrist, back, hip, knees, ankle and
- Any abdominal surgery
- Severe trauma to multiple body parts as a result of a MVA or similar non work related incident.
- The fire fighter who has a medical condition of the respiratory tract may be required to have a pulmonary function test included in their return-to-duty exam.
In accordance with NFPA 1582, Medical Requirements for Fire Fighters, 2000 Edition, a musculoskeletal evaluation will be performed on all current fire fighters at the time of promotion to determine the fire fighter's medical ability to continue participating in a training or emergency operational environment as a fire fighter.
Fire Fighter's Preplacement Medical Evaluation
The fireman exam to be performed on all appropriate candidates for the Fire Fighters positions will include: Complete medical history, Complete occupational history, Vital signs to include height and weight, temperature, pulse, blood pressure and respirations, Physical exam by physician, System review to include dermatological, EENMT, cardiovascular respiratory, GI, GU, Endocrine and metabolic, neurological, MS, Rectal exam with stool guaiac, Vision exam (peripheral, color, visual acuity), Pulmonary Function Test by a NIOSH Certified Clinician, Electrocardiogram, Audiology Exam by a CAQH Certified Clinician, Chest x-ray, Laboratory Studies (Complete Blood Count, Chemistry Panel, Glucose, Creatine, Lipid Panel, liver functions, Electrolytes, Urinalysis), and Musculoskeletal evaluation of the ability to perform the essential functions of the position.
Fire Fighters' Physicals Periodic Medical Evaluation
In accordance with the ADA and NFPA 1582, Medical Requirements for Fire Fighters, 2000 Edition, OHS presents this exam to be performed on all current fire fighters to determine the fire fighter's medical ability to continue participating in a training or emergency operational environment as a fire fighter and will include: Complete medical history, Complete occupational history, Vital signs to include height and weight, temperature, pulse, blood pressure and respirations, Physical exam by physician, Rectal exam with stool guaiac, Vision exam (peripheral, color, visual acuity), Pulmonary Function Test by a NIOSH Certified Clinician, Electrocardiogram, Audiology Exam, Chest x-ray, Laboratory Studies (Complete Blood Count, Chemistry Panel, Electrolytes, Urinalysis).
Fire Fighters' Physicals Return-To-Duty Medical Evaluation
In accordance with the ADA and NFPA 1582, Medical Requirements for Fire Fighters, 2000 Edition, OHS presents this exam to be performed on all appropriate candidates who have been absent from duty for a medical condition of a nature or duration that may affect performance as a fire fighter and will include: Complete medical history, Complete occupational history, Vital signs to include height and weight, temperature, pulse, blood pressure and respirations, physical exam by physician, musculoskeletal evaluation of the ability to perform the essential functions of the position will be performed only if the medical condition caused any impairment to the musculoskeletal system.

Police Officer Examinations
Police Officer's Physicals Preplacement Exam
In accordance with the medical guidelines issued by the Criminal Justice Training Council pursuant to M.G. L. c.6 s. 118, Company Care presents the following medical examination proposal for your review:
Police Officer Training Program:
This exam will be performed on all appropriate candidates for the Police Officer Training Programs and will include: Complete medical history, Complete occupational history, Vital signs to include height and weight, temperature, pulse, blood pressure and respirations, Physical exam by physician, Rectal exam with stool guaiac, Vision exam (peripheral, color, visual acuity), Electrocardiogram, Audiology exam, Chest x-ray, Laboratory studies (Complete blood count, Chemistry panel, T3, T4, TSH, Electrolytes, Serology (RPR), Urinalysis, Tuberculosis (Mantoux), Drug screen) Musculoskeletal evaluation of the ability to perform the essential functions of the position.
Police Officers' Physical Periodic Medical Evaluation

Company Care presents this exam to be performed on all appropriate candidates who have been absent from duty for any condition of a nature or duration that may affect performance as a police officer and will include: Complete medical history, Complete occupational history, Vital signs to include height and weight, temperature, pulse, blood pressure and respirations, Physical exam by physician, Musculoskeletal evaluation of the ability to perform the essential functions of the position will be performed only if the medical condition caused any impairment to the musculoskeletal system.
